Workman’s Compensation & Injury

Dr. Giaquinto has 18 years of experience conducting QME and QME-type evaluation for applicant and defense. An unbiased approach and professionalism are always maintained. Reports are detailed and thorough relying on substantial evidence, causation and reasonable medical probability. Appointments can be scheduled to meet demands. I am able to provide the completed report in a short amount of time.

All tests used in the evaluation are standardized tests that are industry proven to be reliable and valid measures. You are able to request specific tests to be administered. Reports are very detailed with all measures integrated into a comprehensive report with a summary and specific recommendations. All questions from the referral source will be addressed and clearly answered.

C & P (Compensation and Pension) Evaluations

I provide C & P “exams” for veterans to assess their condition for disability benefits. I ensure the veteran has ample time to describe the injury/s and circumstances so that he/she feels they were able to fully express themselves and feel understood.

Competency Evaluations & Fitness for Duty

Conduct such evaluations for corporations, law enforcement agencies, firefighter agencies, railroad, state and federal agencies. These range from initial fit for duty evals, re-appointment, return to duty, or to appeal a denial.

Disability Evaluations

Provide disability evaluations for the state of California in relation to disability benefits. Dr. G also conducts disability evaluations for private companies, corporations, and various government agencies. Evaluations to determine Malingering are also conducted.

IME Evaluations

Approved by the State of California to conduct Independent Medical Evaluations throughout the state. IME evaluations are available to individuals, organizations and firms.

IEP Evaluations

Provide IEP evals for schools as well as for parents and individuals who are in need. Evals are also conducted for students who may be in need of Special Accommodations in school of all grades. Dr. G has performed these evaluations for LAUSD, private schools and students in college.

Pain Evaluations

Evaluations will assess the nature and level of pain and its effects and limitations on the body, ADL’s, mood, sleep and psyche. Objective and subjective measures will be used in the pain assessment.

Pre & Post-Surgical Evaluations

Evaluations for physicians who require their patients to have an evaluation before/after surgery to ensure they are emotional and psychologically ready and stable before and after a particular surgery procedure are performed.

Psychological and Neuropsychological Evaluation

These evaluations are conducted to assess an individual’s psychological, cognitive, emotional, behavioral status. They will determine what symptoms a person is experiencing in regard to intensity, frequency and duration. Strengths and weaknesses will be identified, diagnostic suggestions will be given along with recommendations. Neuropsychological evals will determine whether any organic impairment exists.

Psycho-educational

These evaluations will determine the individuals psychological, emotional and cognitive make-up. They will also determine the individual’s level of achievement – what their actual potential is. Strengths and weaknesses will be identified along with recommendations of how to address any weaknesses or disabilities.
